Consultant somatics/elderly GP practice De Amstel

Do you make the difference for our patients? As a POH at De Amstel, you guide chronically ill patients step by step towards a healthier lifestyle and help vulnerable elderly people maintain their independence and daily structure. With your coaching in health and lifestyle, you give them the confidence to keep control of their own lives for as long as possible.

Your function

As a Practice Support Physician (POH-S/POH-O), you are the lynchpin in the care of our chronic patients and frail elderly. You are responsible for the supervision and support of chronically ill patients and frail elderly people within the practice. You work independently and with a lot of personal responsibility. Your duties consist of:

• You perform consultations independently and draw up personal care plans with patients.
• You coach on lifestyle and self-management so that patients can continue to function independently for as long as possible.
• You identify changes in health and quickly liaise with GPs, fellow POHs and external healthcare providers.

Who are you?

• You have completed training as a POH-S and/or POH-O, or are close to finishing training
• You work as a POH-S and find it interesting to take on tasks of a POH-O or you work as a POH-O.
• You communicate kindly, clearly and empathetically with patients, informal carers and colleagues inside and outside the practice.
• You put the patient first and have an eye for what someone really needs.
• You are independent in your work and also possess strong collaboration skills.

Your workplace

Huisartsenpraktijk De Amstel is located in Gezondheidscentrum Waterlinie in Uithoorn, an innovative and modern centre with short lines of communication between the various disciplines. Quality of care, equal cooperation and personal development are central.

The team consists of GPs, practice support staff, practice assistants and a hostess and is characterised by an open, collegial atmosphere where people work hard and laugh a lot too. There is room for own initiative, the contribution of ideas and further development within your profession. We are a training practice.

Salary and more

• Employment of 18 - 24 hours per week, with working days and start date to be determined by mutual agreement.
• A temporary contract with the prospect of permanent employment.
• A salary that grows with your experience, in accordance with the GP Care collective agreement (scale 7: €3,795 - €4,977 for 38 hours).
• Holiday pay, a 13th month, travel allowance, 25 holidays and 25 PLB hours (for a 38-hour working week).
• Space for continuing education and training, and further development within your role as a POH.
